2024-2025 Course Request Forms

Students entering Bernard Middle School for the 2024-2025 school year must complete an online Course Request Form. The 6th grade, 7th grade and 8th grade forms are available now.

Current 6th and 7th grade students are asked to please complete the form by February 1st.

**Incoming 5th grade families are asked to please complete the 2024-2025 Bernard 6th Grade Online Course Request from by Friday, February 9th.
